Kochurani, a vibrant 55-year-old, suddenly found her world turned upside down when she received the devastating diagnosis of stage 4 lung cancer in February 2024. A cherished wife and mother of two, she had always been the beacon of strength and hope for her family and friends.

The Diagnosis:

It all began innocuously with a persistent dry cough in October 2023. Despite diligent adherence to prescribed medications, the cough lingered, prompting further investigation. A plain CT scan initially revealed pneumonia, leading to a two-week hospitalization for treatment. Yet, the cough persisted, prompting a contrast CT scan, which unveiled a concerning mass growth in her left lung. Subsequent lung biopsy confirmed the dreaded diagnosis: lung adenocarcinoma at an advanced stage.

Further tests, including a PET CT scan, revealed the aggressive nature of the cancer, which had metastasized to her right lung, bones, backbone, collar bone, pelvic regions, and the lining of the liver. Amidst escalating pain levels, she underwent radiotherapy to alleviate suffering while awaiting mutation analysis results.

The mutation test results unveiled a rare variant, ERBB2 Exon 20 Insertion, occurring in only 1-5% of adenocarcinoma cases, presenting a daunting treatment challenge. Recommended treatment protocol involved chemotherapy coupled with immunotherapy as the primary line of defence, followed by targeted therapy as the secondary approach. Each cycle of treatment required once every three weeks is a lifeline for Kochurani, promising a second chance to come back to Normal life.

The Plea:

While the family has exhausted savings and incurred loans for a sum of 6,50,000 INR to cover initial treatment costs, the staggering expenses for ongoing care, totalling 26,40,000 INR, looms large. This financial burden threatens to disrupt Kochurani’s crucial treatment regimen, jeopardizing her fight for a second chance at life.

In this time of dire need, we humbly appeal to the kindness and generosity of compassionate souls. Your contribution, no matter the size, will make an immense difference in Kochurani’s battle against cancer. These symptoms led to the diagnosis of critical heart blockages caused by calcium deposits. Dr. Himabindu Annamraju, a leading expert in maternal-fetal medicine at BirthRight, emphasizes that this recent success story highlights the crucial role played by coordinated clinical support in tackling complex medical conditions during pregnancy. The collaborative effort exemplifies the power of teamwork and specialized care in the realm of maternal health.

The patient, facing the challenges of NCPH, received expert care and attention from a multidisciplinary team. Through meticulous planning, advanced medical interventions, and constant monitoring, the team ensured a safe delivery and the birth of a healthy baby.

Diving into the details, a 30-year-old pregnant woman with Non-Cirrhotic Portal Hypertension (NCPH) came to the Star Hospitals at Financial District when she was 17 weeks into her pregnancy, presenting a complex medical challenge. During her assessment, doctors discovered she had grade 3 varices with RCS, an enlarged spleen, and a low platelet count of just 35,000. She was referred to BirthRight by Rainbow Hospitals for a safe delivery.

The multi-disciplinary team at Rainbow Children’s Hospital and BirthRight by Rainbow along with Star Hospitals, Financial District took on her case, assessing her condition and discussing the associated risks with her. They explained to her that she could face potential complications, including variceal bleeding, spontaneous abortion, and premature delivery.

The patient was prescribed beta blockers and was closely monitored throughout the pregnancy paying special attention to her coagulation levels, fetal heart rate, and the health of her baby.

As the pregnancy progressed, she eventually underwent a cesarean section. As liver conditions impact the bleeding factors and coagulation profile of the patient, this patient was carefully managed with the use of TEG to assess and provide necessary corrections to her coagulation and to control bleeding intra and post-operatively.

“She has been diagnosed with Non-Cirrhotic Portal Hypertension (NCPH) during pregnancy. The blood vessels in the liver become engorged, causing portal hypertension. This condition resulted in her spleen enlargement with dilated blood vessels over the food pipe, known as varices. Such patients are at high risk of bleeding or liver failure, compounded by the added complexity of pregnancy.”, noted Dr. Srinivasa Reddy, Consultant Hepatology & Liver Transplant Physician at Star Hospitals Financial District.

Chennai, May 2nd, 2024 – Apollo Cancer Centres (ACC) advocates raising awareness and addresses the challenges posed by the often-overlooked Primary Immunodeficiency Disorders (PIDs). In commemoration of the World Primary Immunodeficiency Week, observed from April 22nd to 29th, ACC spearheads discussions around this crucial health issue, shedding light on its prevalence and impact.

Primary Immunodeficiency Disorders represent a group of inherited conditions that compromise the body’s immune system, leaving individuals susceptible to infections. Shockingly, a significant portion of individuals with PIDs remain undiagnosed, leading to untreated complications and heightened risks. Despite the widespread impact of PIDs, there is alarmingly high mortality rates among affected infants, particularly those below one-year-old. Undiagnosed cases significantly contribute to this concerning statistic. While PIDs tends to be more severe in children, it can also affect adults, albeit with differing diagnostic challenges. In adults, the road to diagnosis may be prolonged, sometimes spanning up to 25 years from the onset of initial symptoms to an accurate PID diagnosis. This delay underscores the importance of heightened awareness and proactive screening initiatives to ensure timely medical intervention and improved outcomes for individuals affected by PID.

Over 6 million people globally suffer from PIDs, with an alarming 70-90% of cases remaining undiagnosed

In India, where consanguineous marriages are prevalent, the incidence of PIDs is notably high, exacerbating the challenge. It is estimated that there could be more than one million patients with PID in India. Since PID is a genetic condition, there is a chance of every pregnancy being affected. Prenatal diagnosis or testing the foetus at 10 to 11 weeks for pregnancy can help detect whether the foetus is affected or not and families need to be counselled accordingly.

According to WHO (World Health Organization and IUIS (International Immunological Societies), over 120 Primary Immunodeficiency Disorders (PIDs) exist, with antibody deficiencies like CVID (Common Variable Immunodeficiency), and XLA (X-linked Agammaglobulinemia), topping the list.
